# Collect feedback and research

> Build surveys that combine structured answers with open-ended evidence.

## Design for useful answers

Combine:

* a rating;
* a multiple-choice question about the main experience;
* an open question about the reason;
* an optional contact question;
* clear research or contact consent.

## Customer feedback prompt

> Create a feedback form for my new delivery service. Ask for a 1 to 5 rating, what they ordered, whether it arrived on time, the best part, the biggest problem, and whether I may contact them.

## Product validation prompt

> Create a waitlist survey for a new invoicing product. Ask what people use today, their biggest problem, how often it occurs, what they have tried, and their email if they want updates.

## Analyze themes

> Group the visible open-ended answers into themes. Count the responses in each theme, show short supporting evidence, and separate product requests from service complaints. Do not invent missing context.

## Research responsibilities

Explain:

* who is collecting the data;
* why it is collected;
* whether answers are anonymous;
* how long you plan to keep them;
* how someone can ask a question or withdraw when applicable.
